Composition and release form
1 ml of solution for injection contains aprotinin 5000 or 10,000 IU; In ampoules of 10 ml, in the package 5 pcs.
Pharmachologic effect
Mode of action - Hemostatic, antiprotease.
It binds proteolytic enzymes (trypsin, chymotrypsin, kallikrein), incl. Activating fibrinolysis (plasmin).
Pharmacodynamics
Inactivates plasma proteinases (plasmin, kininogenase, trypsin, chymotrypsin), blood cells and tissues. Antiprotease activity determines the effectiveness of the use in acute pancreatitis. Inhibiting plasmin, reduces the fibrinolytic activity of the blood, inhibits fibrinolysis and has a hemostatic effect in coagulopathy. Blockade of kallikrein-kinin system allows to use for the prevention and treatment of various forms of shock (endotoxic, traumatic, hemolytic).
Indications of the Traskolan
Acute pancreatitis, peritonitis, hyperfibrinolytic bleeding, hemorrhagic, traumatic and toxic shock, fat embolism, postoperative bleeding, postoperative pancreatitis.
Contraindications
Hypersensitivity.
Side effects
Erythema, urticaria, bronchospasm, nausea, vomiting, myalgia, lability of AD, rarely - hallucinations, confusion, psychosis.
Interaction
Incompatible with corticosteroids, as well as with infusion solutions containing amino acids and fat emulsions.
Dosing and Administration
IV (intravenously) by stream or drop injection. The dose is selected individually, depending on the patient's condition. Acute pancreatitis: 500,000-1000000 KIE without dilution, at a rate of up to 5 ml / min, then - 100,000-500,000 KIE daily, drip. With the improvement of the general condition of the patient, the dose is gradually reduced. Treatment (if necessary) lasts up to 15 days.
Severe conditions: after bolus administration, it is recommended to continue drip treatment at a rate of 100,000 KIU / h for 8-12 h until a clinical improvement is achieved.
Fibrinolytic bleeding: drip, in a dose of 100,000-200,000 KIE, if necessary - up to 500,000 KIE, depending on the intensity of bleeding.
With the preventive purpose in surgery: 200,000-400,000 KIe before the operation, during and after it, then - for 100,000 KIE for the next 2 days.
Precautionary measures
Before use, a skin test should be performed to identify possible hypersensitivity. They are used with caution in patients who received muscle relaxants 2-3 days before.
