Pharmacological effect of Synkapton

Pharmachologic effect – antimigraine.

Pharmacodynamics

Combination drug whose action is determined by the properties of its constituent parts:

Ergotamine tartrate increases the tone of extracranial vessels, extended during migraine attacks, eliminates the headache.

Caffeine increases the absorption of ergotamine in the intestines and also increases the tone of extracranial vessels.

Dimenhydrinate suppress the vomiting center.

The therapeutic effect is 30 minutes and lasts for 3-6 hours.

Pharmacokinetics

Ergotamine tartrate after oral slowly and incompletely absorbed from the intestine at a dose of 2 mg; Plasma Cmax of 0.36 ng / ml; caffeine formulation in the composition increases its plasma concentration to 0.7 ng / ml. T1 / 2 is the plasma ergotamine 2 hours, 90% of its metabolites excreted in the bile. Caffeine is rapidly absorbed, the Cmax in plasma is reached after 1 hour, T1 / 2 of plasma is 3-7 hours. Return with urine in the form of metabolites. Dimenhydrinate well absorbed, T1 / 2 of plasma is 5-8 hours. It is metabolized in the liver and excreted in the urine as metabolites.

Indications for Synkapton

Relief of migraine and migraine headache vascular genesis (the drug is intended for long-term treatment).

Contraindications for Synkapton

hypersensitivity to one of the components;

Side effect

Drowsiness, fatigue. There may be constipation, allergic reactions, mydriasis, peripheral vasoconstriction, cold feeling in extremities, paresthesia, pain behind the breastbone.

Interaction

When combined with the use of sedatives and hypnotics enhanced the effectiveness of the latter.

Concurrent use with troleandomycin with other macrolides, beta-blockers leads to increased vasoconstriction.

Dimenhydrinate may mask ototoxicity of aminoglycosides when co-administered.

Reduces the effectiveness of heparin.

Dosage and Administration

Inside, drinking plenty of water.

Children over 12 and adults - at the first sign of migraine, 1 tab. If improvement does not occur, then take even 1 table. 30 min. The drug can be repeated every 30 minutes to a maximum daily dose (Table 4.). When previously established individual daily dose of it can be assigned a time when the signs of migraine. Weekly intake should not exceed 10 Table.

Children from 6 to 12 years - 1/2 Table.

Overdose

Symptoms include nausea, vomiting, tachycardia, itching, numbness, pain in the limbs (ischemia), dizziness, respiratory depression.

Treatment: gastric lavage with sodium sulphate, the appointment of activated carbon, the control of blood pressure and heart rate, oxygen inhalation; when arteriospazme - in / in the introduction of vasodilators.

Special Instructions

The patient should always be in possession of the drug, especially in situations that provoke migraine attack: menstruation, weather fluctuations, eye strain, use of other medications (oral contraceptives), food (coffee, cheese, chocolate). It is important to have time to take the drug in the prodromal phase, rather than after which developed migraine attack.

It applies only for the relief of migraine attack, and not for long-term treatment. After a week of application you must take a break of the drug. If frequent migraine attacks, it is necessary to coordinate the multiplicity of the drug with the doctor.
