Pharmacodynamics

Vero-narcap® is a polyvalent inhibitor of proteinases. Inactivates the most important proteinases of blood plasma and tissues (including plasmin, kininogenase, trypsin, chymotrypsin) involved in the formation of withdrawal syndrome with opiate addiction. The increased formation of kinins (bradykinin in blood plasma and kallikrein in tissues), the release of leukocyte elastase, neutrophil proteinases, proinflammatory cytokines, causing microcirculation disorders, vasodilation, increased vascular permeability is an important pathophysiological mechanism accompanying the onset of pain syndrome in opium withdrawal syndrome. The mechanism of action of Vero-narcap with opium withdrawal syndrome is, to the greatest extent, connected with the effect on the catabolism system of peptide molecules, which leads to the reduction or reduction of vegetative and pain manifestations of opiate withdrawal syndrome.

Pharmacokinetics

Contraindications

Hypersensitivity;

Sharply expressed allergic reactions (including on the protein of cattle);

DIC-syndrome;

Side effects

From the cardiovascular system: a decrease in blood pressure, tachycardia.

From the side of the central nervous system: psychotic reactions, hallucinations, confusion.

Interaction

Vero-narcap® is pharmaceutically incompatible with other drugs, except for electrolyte solutions and dextrose.

Vero-narcap® should not be combined with β-lactam antibiotics.

Vero-narcap® lowers the activity of fibrinolytic agents, including Streptokinase, urokinase, alteplase; Enhances the effect of heparin.

When combined with Vero-narcap® and a solution of dextran, the risk of undesirable effects increases.

Dosing and Administration

IV (the contents of the vial are diluted in 2 ml of isotonic sodium chloride solution), in a dose of 30000-60000 ATPE once a day, drip (in 100-200 ml of isotonic sodium chloride solution) or slowly slowly (in 20-50 ml of isotonic sodium solution Chloride).

Treatment is carried out before the normalization of the clinical picture (usually within 1-3 days, in some cases - up to 7 days).

Special instructions

Vero-narcap® is more effective in the prevalence of pain and autonomic symptoms in the structure of the withdrawal syndrome.

If side effects occur during administration, discontinue administration immediately.

With DIC, the appointment of Vero-narcap® is possible only after elimination of all manifestations of ICE and against the background of preventive administration of heparin.

Especially careful should appoint Vero-narcap® to patients who in the previous 2-3 days were prescribed muscle relaxants.

For the prevention and treatment of allergic reactions, it is possible to prescribe antihistamines.
