Pharmacological Properties of Vasocardin
Pharmachologic effect - antianginal, antihypertensive, anti-arrhythmic.
Selectively blocking beta1-adrenergic receptors
Pharmacodynamics
It reduces myocardial oxygen consumption, lengthens refractory phase, lowers systolic (arterial hypertension) and diastolic (after a few weeks of therapy), blood pressure, reduces plasma renin activity.
Suppress automaticity of sinus node, slows heart rate slows AV conduction, reduces the excitability of myocardial contractility and cardiac output.
Pharmacokinetics
When ingestion absorbed 95%. Bioavailability is 50%. Binding to plasma proteins - 10%. The volume of distribution - 5.5 l / kg. Penetrates into breast milk. T1 / 2 -. 3.5 hours excreted by the kidneys (95%), mainly as metabolites (only 10% - unchanged).
Indications for Vasocardin
Hypertension, cardiac hyperkinetic syndrome, myocardial infarction, hyperthyroidism, arrhythmia. Prevention of angina attacks, migraine.
Contraindications for Vasocardin
Hypersensitivity. AV block, cardiogenic shock, decompensated heart failure, peripheral vascular disease, sinus bradycardia, chronic obstructive pulmonary disease, labile insulin-dependent diabetes mellitus, metabolic acidosis.
Side effect of Vasocardin
Diarrhoeal phenomenon, headache, dizziness, fatigue, insomnia, depression, shortness of breath, bradycardia, hypotension, cardiac insufficiency, Raynaud's syndrome, blurred vision, allergic reactions.
Interaction
It enhances the effect of antihypertensive and hypoglycemic drugs. Antiarrhythmic agents deepen hypotension and bradycardia, tricyclic antidepressants, barbiturates, nitroglycerin, diuretics, oral contraceptives - only anti-hypertensive effect. Rifampicin reduces, cimetidine increases the concentration in the blood. Increases in blood alcohol concentration and prolongs its action.
Dosage and Administration
Inside (after eating the same time), 100 mg (200 mg) per day in 2 divided doses.
Myocardial infarction: a maintenance dose of 200 mg / day (morning and evening).
In hyperthyroidism: 150-200 mg / day in 3-4 divided doses.
The maximum daily dose of 400 mg.
Overdose
Symptoms: hypotension, congestive heart failure, cardiogenic shock, bradycardia, difficulty breathing, bronchospasm, vomiting, loss of consciousness, generalized convulsions.
Treatment: symptomatic. If bradycardia and conduction disturbances shown in / bolus administration of atropine at a dose of 0.5-2 mg; reducing myocardial contractility - in / glucagon at an initial dose of 1-10 mg, then - 2-2.5 mg / h infusion; seizures - slow in / diazepam.
Precautionary measures
It should be remembered that vasocardin adversely affect the ability to drive and perform work that requires a high rate of mental and physical reactions.
Storage conditions of Vasocardin
In a dry, dark place at a temperature of 15-21 ° C.
Keep out of the reach of children.
Shelf life
2 years.
Do not use beyond the expiration date printed on the package.
