Composition and release form

1 bottle with 5 ml solution for injection contains heparin sodium 25000 IU; In the box 1 bottle.

Pharmachologic effect

Mode of action - Fibrinolytic, antiaggregational, anticoagulant.

The complex with antithrombin III inhibits thrombus formation: binds the activated factor X and inhibits the conversion of prothrombin to thrombin. In large doses, inactivates thrombin and reduces the conversion of fibrinogen into fibrin, prevents the activation of fibrin-stabilizing factor.

Indications for Heparin-Richter

Thrombosis (including coronary vessels complicated by myocardial infarction), thromboembolism (prevention and treatment), disseminated intravascular coagulation.

Contraindications

Hypersensitivity, hypocoagulation, ulcerative lesions and tumors of the gastrointestinal tract, subacute bacterial endocarditis, severe violations of the liver and kidney, operations on the brain and spine.

Pregnancy and breast-feeding

It is possible only on strict indications.

Side effects

Bleeding from mucous membranes and wounds, thrombocytopenia, allergic reactions (bronchospasm, fever, lacrimation, rhinitis, urticaria).

Interaction

The effect is enhanced by other anticoagulants and NSAIDs, weakened by ACTH, alkaloids, thyroxine, tetracyclines. It binds and is inactivated by amines (protamine) and polypeptides having an alkaline reaction.

Dosing and Administration

IV, adults - 15 IU / kg / h, before infusion - 5000 IU (for quick effect); The daily dose should not exceed 60000-80000 IU and only in exceptional cases can be administered more than 10 days. SC (in the abdomen) - 10,000 IU 4 times a day (for medical purposes) or 5000 IU twice a day (with a prophylactic goal). Children: IV, initial dose - 50 IU / kg, then continuously 20000 IU / m2 body surface per day or periodically 100 IU / kg every 4 hours.

Overdose

Symptoms: bleeding of varying severity.

Treatment: with a slight bleeding - to reduce the dose or stop the use of the drug, in case of severe bleeding IV slowly injected 1% solution of protamine sulfate.
