Pharmacotherapeutic group:
Ophthalmic agents in combination
Enzymes and in combinations antienzymes
Beta-blockers in combination
Pregnancy and breast-feeding
Category effects on the fetus by FDA - C.
Pharmacological action
Combined preparation has antiglaucoma action. Each of the components reduces intraocular pressure by reducing the secretion of aqueous humor. Dorzolamide - carbonic anhydrase inhibitor; reduces intraocular fluid secretion (slowing down the formation with consequent weakening hydrogencarbonate Na + transport and water). There is no accommodation spasm, cramps, night blindness. Timolol - nonselective beta-adrenoceptor blocker with little intrinsic AGR. When applied topically, it lowers intraocular pressure by reducing aqueous humor formation and a slight increase in its outflow.
Indications
Otkratougolnaya (including secondary) glaucoma; pseudoexfoliation glaucoma; ocular hypertension.
Contraindications
Hypersensitivity, bronchial asthma, bronchospasm, bradycardia, AV block II-III century., CHF II-III century., Cardiogenic shock, renal failure (creatinine clearance less than 30 ml / min).
Precautions
Liver failure, pregnancy, children's age.
Dosing
Buried 1 cap in the conjunctival sac, 2 times a day. If the treatment is applied by several local ophthalmic drugs, drug administration should occur at intervals of 10 minutes.
Side effect
Dorzolamide: burning, crawling, itchy eyes, watery eyes, blurred vision, irritation of the eyelid, superficial punctate keratitis, conjunctivitis, myopia, blepharitis; rarely - iridocyclitis. Allergic reactions, bitter taste in the mouth, nausea, headache, asthenia.
Overdose
Symptoms: violation of water-electrolyte metabolism, acidosis, disorders of the central nervous system.
Treatment: symptomatic.
Timolol: conjunctival hyperemia, tearing, photophobia, corneal epithelium edema, burning and itching in the eyes, skin hyperemia century, diplopia, blepharitis, keratitis, decreased corneal sensitivity, ptosis. With prolonged use at high doses are possible systemic side effects.
Interaction
Antiglaucoma drugs (beta-blockers, pilocarpine, dipivefrin, carbachol) enhance the effect. With simultaneous use of carbonic anhydrase inhibitors and beta-blockers into the possible additive effect with regard to systemic effects.
Special instructions
In the period of treatment should regularly measure the intraocular pressure control slezovydeleniya function integrity of the cornea, the visual field - at least 1 time in 6 months.
When long-term therapy is necessary to monitor the concentration of K +, serum electrolytes, blood pH of, heart rate, blood pressure, etc. Parameters of cardiac activity. Be careful when driving at night, to limit the activities in low light. The drug can be used patients who take hard contact lenses made of polymethylmethacrylate. The preservative contained in the product may be delayed for soft contact lenses, so lenses should be removed prior to instillation and put on no earlier than 15 minutes after it.
In the case of the upcoming surgery with general anesthesia, it is necessary to cancel the drug 48 hours before the operation, because timolol increases the effects of muscle relaxants and general anesthetics.
