Pharmacological Properties
Pharmachologic effect

Beta-adrenoceptor blocking.

Supraventricular tachyarrhythmias (tachycardia of the AV connection, supraventricular tachycardia with WPW-syndrome, paroxysm of atrial fibrillation), ventricular tachycardia.

Hypersensitivity (including to sulfonamides), congestive heart failure III-IV degree, shock, AV block II and III degree sinoatrial block, sick sinus syndrome, bradycardia, hypokalemia, hypotension, elongated the QT interval, occlusive peripheral arterial disease obstructive pulmonary disease, metabolic acidosis, laryngeal edema, allergic rhinitis.

Pregnancy and breast-feeding

Allowed only for health reasons (for 48-72 hours prior to delivery is recommended to stop taking).

Side effects

Headache, dizziness, drowsiness, fatigue, cold extremities; rarely - a violation of the digestive tract function, bronchospasm, skin reactions, insomnia, increased severity of heart failure, bradycardia, AV conduction disorders, hypotension; decrease in lacrimation, hypoglycemia, hallucinations, dry mouth, muscle cramps, transient impotence, violation of visual acuity, conjunctivitis.

In combination with nifedipine, tricyclic antidepressants, barbiturates, phenothiazine derivatives, drugs, vasodilators, diuretics and other antihypertensive drugs cause a significant reduction, with noradrenaline and MAO inhibitors - a sharp increase in blood pressure. Drugs and anti-inflammatory drugs increase cardiodepressivny effect. Could enhance neuromuscular blockade against the backdrop of tubocurarine. Reserpine, clonidine, alpha-methyldopa, guanfacine, cardiac glycosides increase the severity of the negative chrono and dromotropic action. It increases the effect of insulin and oral hypoglycemic agents (especially against physical stress).

Dosing and Administration

Inside, the food with a small amount of water - 160 mg (1 tablet of 160 mg or 2 mg Table 80..), 2 times a day, or 80 mg (1 tablet of 80 mg.) 2-3 times a day; Cl patients with creatinine in the range 10-30 ml / min, the dose is reduced by half; Cl creatinine at least 10 ml / min 4.1 recommended use dosage. B / administered in a single dose of 20 mg, repeated administration is possible in 20 minutes.

Symptoms: fatigue, dilated pupils, loss of consciousness, generalized myoclonic seizures, hypotension, bradycardia (with asystole), ventricular tachycardia (atypical), symptoms of cardiogenic and hypovolemic shock.

Treatment: removal of the drug from the body (gastric lavage, hemodialysis); atropine, dopamine, isoprenaline, adrenaline, glucagon indicated.

Precautionary measures

Not recommended simultaneous reception hinidinpodobnymi antiarrhythmic drugs. Be wary appoint diabetes, pheochromocytoma, disorders of the kidneys and liver, myocardial infarction (in history), psoriasis, in patients who are on a diet. abrupt discontinuation of treatment should be avoided (perhaps the "cancellation" syndrome).
