Pharmacological action.

Combined preparation containing corticosteroids and selective beta2-agonists. Budesonide - GCS high local and low systemic pharmacological activity. It has anti-allergic, anti-inflammatory and antiekssudativnoe action that inhalation route of administration results in a reduction of bronchial obstruction. The mechanism of action is inhibition of the release of mediators of inflammation and allergies, as well as to reduce the reactivity of the smooth muscle of the bronchi. Budesonide prevents the development of asthma attacks, but relieves bronchospasm. Therapeutic effect develops gradually over 7-10 days. Salbutamol - a selective beta2-adrenergic stimulant receptors. Has bronchodilatory effect, prevents and relieves bronchospasm, reduces the resistance in the respiratory tract, increases the VC. It prevents the release of histamine and leukotrienes from mast cells. Bronchodilator effect occurs rapidly within 1-3 minutes after inhalation, and lasts for 5-7 hours after administration of a single dose. virtually no chronotropic and inotropic effects in therapeutic doses.

Indications.

Bronchial asthma.

Contraindications.

Hypersensitivity, pulmonary tuberculosis (active phase), fungal, viral, bacterial infections of the respiratory organs, pregnancy (I-II trimester), threatening miscarriage, children's age (16 years).

Precautions

Arrhythmias (especially tachyarrhythmias), myocarditis, heart disease (including aortic stenosis), hypokalemia, hypertension, acute heart failure, diabetes, hyperthyroidism, glaucoma, pregnancy, lactation.

Dosage

Inhalation. Titration drugs included in the drug occurs individually depending on the severity of the disease. The recommended dose of - 1 3 inhalations twice a day or 2 times 2 inhalations per day.

After achieving optimal control of asthma symptoms in patients receiving the drug may reduce the daily dose to minimum effective maintenance dose until receiving one time per day.

Side effect.

Irritation of the mucous membranes of the mouth and throat, coughing, hoarseness (dysphonia), and Athos, thrush and throat, dryness and bad taste in the mouth, nausea, vomiting.

Rarely - anxiety, tremor of skeletal muscles, headache, decreased blood pressure, collapse, palpitations, paradoxical bronchospasm, skin rash, increased nervous irritability, depression, sleep disorders, behavioral disorders, systemic side effects of corticosteroids.

Overdose.

Symptoms: tachycardia, weakness, decreased blood pressure, tremor.

Treatment: symptomatic, the introduction of cardioselective beta-blockers.

Interaction.

Not established clinically significant interactions of budesonide, a component of the drug, on the other. Drugs used in the treatment of asthma. However, caution should be exercised with concomitant administration of inhibitors of the enzyme system cytochrome CYP3A4 (ketoconazole, ritonavir) as may develop systemic effects of budesonide.

Not compatible with non-selective beta-blockers.

Xanthine derivatives, corticosteroids, diuretics - the risk of hypokalemia.

Special instructions.

Not intended for initial selection of therapy in the early stages of treatment of asthma.

Hoarseness, coughing, resulting from settling of the drug in the oropharynx as a result of incorrect inhalation technique typically tested after reducing the dose.

Local infections caused by yeast fungi, quickly cease to local antifungal therapy without interrupting drug treatment.

To prevent the development of local candidiasis of the oral cavity and pharynx must be good rinse the mouth with water after inhalation without swallowing.

Switching from systemic corticosteroids to inhaled drug can cause allergic reactions such as rhinitis and eczema, which were previously suppressed by the systemic corticosteroids. When these symptoms should be the appointment of antihistamine drugs.

During the transition from systemic corticosteroids to inhaled patients may experience fatigue, headache, arthralgia and myalgia, and sometimes nausea and vomiting.
