Characteristics of Phenylbutazone
NSAIDs (non-steroidal anti-inflammatory drugs), a derivative of pyrazolone. White or white with a slightly yellowish tinge crystalline powder. Virtually nerastvorim in water and acid solutions, it is difficult to dissolve in ethanol, soluble in a solution of sodium hydroxide, chloroform, ether, acetone.
Pharmacology
Pharmacological action - anti-inflammatory, antipyretic, analgesic.
Indiscriminately inhibits the activity of cyclooxygenase (COX (cyclooxygenase) -1 and COX-2) and reduces synthesis of PG. Reduces the level of uric acid in the blood.
When ingestion is rapidly absorbed from the digestive tract (gastrointestinal tract). Cmax achieved after 1-2 hours Bioavailability - 85-90%, decreases with prolonged admission. Binding to plasma proteins is more than 95%. In the liver, biotransformed with the formation of two major metabolites. In significant amounts excreted in breast milk. T1 / 2 - 18-21 hours Excreted in the urine 70% (5% - unchanged), through the gastrointestinal tract - 30% (in the form of metabolites).
Application of Phenylbutazone
For systemic use: inflammatory and degenerative diseases of the musculoskeletal system - rheumatoid arthritis, gouty, psoriatic arthritis, ankylosing spondylitis (Bechterew's disease), osteoarthritis, arthritis in systemic lupus erythematosus (as part of complex therapy). Pain syndrome: bursitis, tendovaginitis, arthralgia, myalgia, neuralgia, migraine, dental and headache, algodismenorea; pain with injuries, burns. Feverish syndrome ("colds" and infectious diseases).
For topical application: articular syndrome with exacerbation of gout, rheumatoid arthritis, osteoarthritis, ankylosing spondylitis. Pain syndrome (mild and moderate intensity) - arthralgia, myalgia, neuralgia, phantom pain; Dermatitis caused by various chemical and mechanical effects, burns (I and II degrees of a small area, including solar ones), inflammation of the skin in places with / m and / in injections, insect bites, superficial thrombophlebitis, inflammation of hemorrhoids, traumatic injuries of soft tissues, inflammatory infiltrates, hematoma, stretching of muscles and tendons, bursitis, tendovaginitis.
Contraindications
Hypersensitivity. For oral administration: "aspirin" triad (combination of bronchial asthma, recurrent nasal polyposis and paranasal sinuses and intolerance to acetylsalicylic acid and drug (drug) pyrazolone series), erosive and ulcerative lesions of the gastrointestinal tract in the stage of exacerbation (peptic ulcer and duodenal ulcer, ulcerative colitis, peptic ulcer), bone marrow depression, decompensated chronic heart failure or pulmonary heart failure, severe hepatic and / or renal insufficiency.
For topical application: trophic ulcers, violation of the integrity of the skin, eczema.
Restrictions for use
For oral administration: hyperbilirubinemia, bronchial asthma, chronic heart failure, edema, arterial hypertension, hemophilia, hypocoagulation, hepatic insufficiency, chronic renal failure, hearing loss, vestibular apparatus pathology, blood diseases, diabetes mellitus, systemic lupus erythematosus, rheumatic polymyalgia, transient arteritis, inflammatory and ulcerogenic diseases of the digestive tract, stomatitis.
pregnancy and lactation
With caution in pregnancy and lactation.
Side effects of Phenylbutazone
Ingestion
On the part of the intestine: dyspepsia, NSAIDs (non-steroidal anti-inflammatory drugs) -gastropathy, abdominal pain, nausea, vomiting, heartburn, diarrhea / constipation; with prolonged use in large doses - ulceration of the mucous membrane of the gastrointestinal tract, aphthous stomatitis, glossitis, erosive esophagitis, bleeding from the gastrointestinal tract, incl. gingival, hepatic dysfunction.
From the cardiovascular system and blood (hematopoiesis, hemostasis): increased blood pressure, tachycardia, agranulocytosis, leukopenia, anemia, thrombocytopenia.
From the nervous system and sensory organs: headache, dizziness, drowsiness, depression, agitation, hearing loss, tinnitus.
Allergic reactions: skin rash, itching, urticaria, bronchospasm, edematous syndrome, angioedema.
Other: increased sweating, uterine bleeding.
With topical application: allergic reactions (including skin rash).
Interaction
Phenylbutazone enhances the effect of indirect anticoagulants, antiplatelet agents, fibrinolytic agents. It can delay the excretion of various drugs (including morphine, PASK (para-aminosalicylic xylitol), penicillin, oral anticoagulants, antidiabetic drugs) kidneys, promote their accumulation in the body and can cause side effects. Increases the concentration in the blood of drugs Li +, methotrexate. Phenylbutazone reduces the effectiveness of uricosuric, hypotensive drugs and diuretics. Antacids and colestyramine reduce absorption. Myelotoxic drugs increase the manifestation of hematotoxicity.
Overdose
Symptoms: cyanosis of fingertips, lips, skin, dizziness, headache, increase / decrease in blood pressure (BP), hyperventilation of the lungs, clouding of consciousness; in children - myoclonic cramps, nausea, vomiting, abdominal pain, hemorrhages, impaired liver and kidney function.
Treatment: gastric lavage, the introduction of activated charcoal, symptomatic therapy, maintenance of vital body functions. Forced diuresis and hemodialysis are ineffective.
Routes of administration
Inside, externally.
Precautions for Phenylbutazone
During treatment, it is necessary to monitor the picture of peripheral blood and the functional state of the liver and kidneys. If it is necessary to determine 17-ketosteroids, the drug should be discontinued 48 hours before the test.
During treatment, one should refrain from engaging in potentially dangerous activities that require increased attention and speed of mental and motor reactions.
Ointment is not used to treat deep vein thrombophlebitis. In the case of therapy for superficial thrombophlebitis, it should be noted that the drug does not replace therapy with anticoagulants. With prolonged application of the drug to the injured skin, it is recommended to conduct a control study of the peripheral blood picture every 2 to 4 weeks.
