Code CAS 15687-27-1

Characteristics of Ibuprofen

Ibuprofen is a racemic mixture of S- and R-enantiomers. White or off-white crystalline powder, practically insoluble in water, highly soluble in organic solvents (ethanol, acetone). Molecular weight 206.28.

Pharmacology

Pharmacological action - anti-inflammatory, antipyretic, analgesic.

Nekelectively inhibits COX-1 and COX-2, reduces the synthesis of PG. The anti-inflammatory effect is associated with a decrease in vascular permeability, an improvement in microcirculation, a decrease in the release of inflammatory mediators (PG, kinin, LT) from the cells and the suppression of energy supply of the inflammatory process. Analgesic effect is due to a decrease in the intensity of inflammation, a decrease in the production of bradykinin and its algogenicity. In rheumatoid arthritis it affects mainly the exudative and partly the proliferative components of the inflammatory reaction, has a quick and pronounced analgesic effect, reduces puffiness, morning stiffness and limited mobility in the joints. Reducing the excitability of the heat-regulating centers of the intermediate brain is resultant in antipyretic action. The severity of the antipyretic effect depends on the initial body temperature and dose. With a single admission, the effect lasts up to 8 hours. With primary dysmenorrhea reduces intrauterine pressure and the frequency of uterine contractions. Reversibly inhibits platelet aggregation.

Since GHGs delay the closure of the arterial duct after birth, it is believed that COX suppression is the main mechanism of action of ibuprofen for IV application in newborns with an open arterial duct.

When ingestion is well absorbed from the digestive tract. Cmax is created within 1 hour, after taking food after 1.5-2.5 hours. Binding to plasma proteins is 90%. Slowly penetrates into the joint cavity, but it lingers in the synovial tissue, creating in it greater concentrations than in the plasma. Biological activity is associated with the S-enantiomer. After absorption, about 60% of the pharmacologically inactive R-form is slowly transformed into an active S-form. Undergoing biotransformation. There are 3 main metabolites excreted by the kidneys. Unchanged in the urine excreted not more than 1%. It has a two-phase elimination kinetics with T1 / 2 from plasma of 2-2.5 h (for retard forms - up to 12 h).

From the digestive tract: obstruction and perforation of the intestine.

On the part of the kidneys: a decrease in the volume of urine formed, the presence of blood in the urine.

At present, there are data on approximately 1,000 preterm infants found in the ibuprofen literature and obtained in clinical studies of ibuprofen with an iv administration. The causes of adverse events observed in preterm infants are difficult to estimate, they can be related both to the hemodynamic consequences of the open arterial duct, and to the direct effects of ibuprofen.

The following unfavorable events are listed below:

From the hematopoietic and lymphatic system:> 1/10 - thrombocytopenia, neutropenia.

From the nervous system:> 1/100, <1/10 - intraventricular hemorrhage, periventricular leukomalacia.

On the part of the respiratory system, chest and mediastinal organs:> 1/10 - bronchopulmonary dysplasia; > 1/100, <1/10 - pulmonary hemorrhage; > 1/1000, <1/100 - hypoxemia (occurred within 1 hour after the first infusion with normalization of the state within 30 min after inhalation of nitric oxide).

From the side of the kidneys and urinary tract:> 1/100, <1/10 - oliguria, fluid retention, hematuria; > 1/1000, <1/100 - acute renal failure.

Gastrointestinal disorders:> 1/100, <1/10 - necrotizing enterocolitis, perforation of the intestine; > 1/1000, <1/100 - gastrointestinal bleeding.

Other:> 1/10 - increase in the concentration of creatinine in the blood, a decrease in the concentration of sodium in the blood.

With external application

Irritation of skin in the form of redness, swelling, rashes or itching; with long-term use - systemic adverse reactions.

Interaction

It should avoid the simultaneous use of two or more NSAIDs due to an increased risk of side effects. With the simultaneous use of ibuprofen reduces the anti-inflammatory and antiaggregatory effect of acetylsalicylic acid (possibly increasing the incidence of acute coronary insufficiency in patients receiving low doses of acetylsalicylic acid as antiplatelet agent after taking ibuprofen). When used with thrombolytic drugs (alteplase, streptokinase, urokinase), the risk of bleeding increases at the same time. Ibuprofen increases the effect of indirect anticoagulants, antiaggregants, fibrinolytic drugs.

Inductors of microsomal oxidation (phenytoin, ethanol, barbiturates, rifampicin, phenylbutazone, tricyclic antidepressants) increase the production of hydroxylated active metabolites, increasing the risk of developing severe hepatotoxic reactions. Inhibitors of microsomal oxidation reduce the risk of hepatotoxic action.

Antacids and colestyramine reduce the absorption of ibuprofen. Caffeine increases the analgesic effect of ibuprofen. Cefamandol, cefoperazone, cefotetan, valproic acid increase the frequency of hypoprothrombinemia.

Myelotoxic drugs increase the manifestation of hematotoxicity of ibuprofen. Cyclosporine and gold preparations increase the effect of ibuprofen on the synthesis of PG in the kidneys, which is manifested by increased nephrotoxicity. Ibuprofen increases the plasma concentration of cyclosporine and the likelihood of its hepatotoxic effects. LS, blocking tubular secretion, reduce excretion and increase the plasma concentration of ibuprofen.

Ibuprofen reduces the effect of antihypertensive drugs (including BCC and ACE inhibitors), natriuretic and diuretic activity of furosemide and hydrochlorothiazide, the effectiveness of uricosuric drugs. Increases the side effects of mineralocorticoids, glucocorticoids, estrogens, ethanol. Strengthens the effect of oral hypoglycemic drugs and insulin. Increases the concentration in the blood of digoxin, lithium and methotrexate. Ibuprofen may reduce the clearance of aminoglycosides (with the simultaneous appointment may increase the risk of nephrotoxicity and ototoxicity).

Ibuprofen for intravenous administration should not be confused with other drugs.

Overdose

Symptoms: abdominal pain, nausea, vomiting, retardation, drowsiness, depression, headache, tinnitus, metabolic acidosis, coma, acute renal failure, hypotension, bradycardia, tachycardia, atrial fibrillation and respiratory arrest.

Treatment: gastric lavage (only for the first hour after ingestion), activated charcoal (to reduce absorption), alkaline drink, forced diuresis and symptomatic therapy (correction of KHS, AD, gastrointestinal bleeding).

Routes of administration

Inside, externally, rectally (children 3 months-2 years), in / in (newborn).

Precautions for Ibuprofen

Treatment with ibuprofen should be carried out at the lowest effective dose, with the minimum possible short course. During long-term treatment, control of the peripheral blood picture and the functional state of the liver and kidneys is necessary.

Given the possibility of developing NSAID-gastropathy, with caution appoint elderly people, in the presence of an anamnesis of stomach ulcers and other gastrointestinal diseases, gastrointestinal bleeding, with simultaneous therapy with glucocorticoids, other NSAIDs and for long-term therapy. When symptoms of gastropathy appear, careful monitoring (including esophagogastroduodenoscopy, a blood test with determination of hemoglobin, hematocrit, analysis of feces for latent blood) is shown. To prevent the development of NSAIDs, gastropathy is recommended to be combined with PGE (misoprostol) preparations.

With caution appoint patients with violations of the liver and kidneys (regular monitoring of bilirubin, transaminase, creatinine, concentration of kidneys), arterial hypertension and chronic heart failure (daily control of diuresis, body weight, blood pressure) is mandatory. If visual impairment occurs, reduce the dose or cancel the drug.

It is not recommended to drink alcoholic beverages during the treatment period.

Do not apply to an open wound surface, damaged skin; avoid getting cream, gel or ointment in the eyes and mucous membranes.
